Next thing you know, it’s dinnertime, and you haven’t finished the homework you started two hours before.

That’s what it’s like to have Attention Deficit Disorder. I know because I’ve had ADHD for as long as I can remember. For me, ADHD means that I can’t focus whenever I really need to. It’s something I will live with for the rest of my life. And it’s no fun!

When I was younger, people told me I was really smart. But I never got good grades to show it. When I was at school, I would get bored really quickly. Then I would look for something more interesting to do. Sometimes I would try to help other kids with their work. The problem was, I didn’t finish my work, and that would lead to trouble. There were lots of days I even felt like quitting school.

My parents were confused. They knew I was smart, but I wasn’t showing it. My doctor suggested that I see a specialist. He gave me a bunch of tests. When it was all done, he told my parents that I had ADD . Now it’s called ADHD. The H stands for “hyper.” He said I didn’t really have the H , so I guess that was some good news.

To help me focus, the doctor gave me some tips to follow. One of them is to keep a special journal with me all the time to write down things, like what homework I have and when things are due. I try to keep the notebook with me wherever I go. It really helps.

I came up with another tip myself. When I have a test or a quiz, I challenge myself to get it done by a certain time. That keeps me focused on the test and not on the pretty girl sitting in front of me or the lizard in the aquarium. Ah, lizards. I really like lizards. Where was I again?

Oh yeah, my focus techniques. With the help of my parents and my older brother, I started doing some other things that help, like going to bed a little earlier so I can get a good night’s sleep.

My brother and I share a bedroom, and he has agreed to go to bed earlier to help me out. Another thing our whole family has started doing is eating a healthy diet. I used to eat a lot of junk food, but now I only eat a little bit. Ah, junk food. Oops, I’ll try not to do that again.

I’ve been working hard, using these focus techniques for the last year and guess what? My grades have started to go up! In fact, on my last report card I got five As and one B. That’s the best I’ve ever done!

My teacher, Miss Ryan, suggested I write this essay. I’m not sure if I knew who Jackie Robinson was before this, but I did some checking. Turns out, he was a great man who had to overcome one of the worst things there is: racism. He did it using the values of courage, determination, teamwork, persistence, integrity, citizenship, justice, commitment, and excellence.

I have used some of these same values to help me overcome ADHD. For instance, I am committed to using my focus techniques, and I am determined to do better in school. Plus, my family has helped me, and that is being a team. Go, team! Also, when I focus, I am a good citizen and don’t bother my classmates as much. Last but not least, using these values has helped me to get almost all As on my report card, which is an example of excellence. Thanks for being such a good example, Jackie!

How to Tackle an Essay (an ADHD-friendly Guide)

6 steps and tips.

Most of the college students I work with have one major assignment type that gets them stuck like no other: the dreaded essay. It has become associated with late nights, requesting extensions (and extensions on extensions), feelings of failure, and lots of time lost staring at a screen. This becomes immensely more stressful when there is a thesis or capstone project that stands between you and graduation.

The good news?

An essay doesn’t have to be the brick wall of doom that it once was. Here are some strategies to break down that wall and construct an essay you feel good about submitting.

Step 1:  Remember you’re beginning an essay, not finishing one.

Without realizing it, you might be putting pressure on yourself to have polished ideas flow from your brain onto the paper. There’s a reason schools typically bring up having an outline and a rough draft! Thoughts are rarely organized immediately (even with your neurotypical peers, despite what they may say). Expecting yourself to deliver a publishing-worthy award winner on your first go isn’t realistic. It’s allowed to look messy and unorganized in the beginning! There can be unfinished thoughts, and maybe even arguments you aren’t sure if you want to include. When in doubt, write it down.

Step 2: Review the rubric

Make sure you have a clear understanding of what the assignment is asking you to include and to focus on. If you don’t have an understanding of it, it’s better to find out in advance rather than the night before the assignment is due. The rubric is your anchor and serves as a good guide to know “when you can be done.” If you hit all the marks on the rubric, you’re looking at a good grade.

I highly recommend coming back to the rubric multiple times during the creative process, as it can help you get back on track if you’ve veered off in your writing to something unrelated to the prompt. It can serve as a reminder that it’s time to move onto a different topic - if you’ve hit the full marks for one area, it’s better to go work on another section and return to polish the first section up later. Challenge the perfectionism!

Step 3: Divide and conquer

Writing an essay is not just writing an essay. It typically involves reading through materials, finding sources, creating an argument, editing your work, creating citations, etc. These are all separate tasks that ask our brain to do different things. Instead of switching back and forth (which can be exhausting) try clumping similar tasks together.

For example:

Prepping: Picking a topic, finding resources related to topic, creating an outline

Gathering: reading through materials, placing information into the outline

Assembling: expanding on ideas in the outline, creating an introduction and conclusion

Finishing: Make final edits, review for spelling errors and grammar, create a title page and reference page, if needed.

Step 4: Chunk it up

Now we’re going to divide the work EVEN MORE because it’s also not realistic to expect yourself to assemble the paper all in one sitting. (Well, maybe it is realistic if you’re approaching the deadline, but we want to avoid the feelings of panic if we can.) If you haven’t heard of chunking before, it’s breaking down projects into smaller, more approachable tasks.

This serves multiple functions, but the main two we are focusing on here is:

  • it can make it easier to start the task;
  • it helps you create a timeline for how long it will take you to finish.

If you chunk it into groups and realize you don’t have enough time if you go at that pace, you’ll know how quickly you’ll need to work to accomplish it in time.

Here are some examples of how the above categories could be chunked up for a standard essay. Make sure you customize chunking to your own preferences and assignment criteria!

Days 1 - 3 : Prep work

  • ‍ Day 1: Pick a topic & find two resources related to it
  • Day 2: Find three more resources related to the topic
  • Day 3: Create an outline

Days 4 & 5 : Gather

  • ‍ Day 4: Read through Resource 1 & 2 and put information into the outline
  • Day 5: Read through Resource 3 & 4 and put information into the outline

Days 6 - 8 : Assemble

  • ‍ Day 6: Create full sentences and expand on Idea 1 and 2
  • Day 7: Create full sentences and expand on Idea 3 and write an introduction
  • Day 8: Read through all ideas and expand further or make sentence transitions smoother if need be. Write the conclusion

Day 9: Finish

  • ‍ Day 9: Review work for errors and create a citation page

Hey, we just created an outline about how to make an outline - how meta!

Feel like even that is too overwhelming? Break it down until it feels like you can get started. Of course, you might not have that many days to complete an assignment, but you can do steps or chunks of the day instead (this morning I’ll do x, this afternoon I’ll do y) to accommodate the tighter timeline. For example:

Day 1: Pick a topic

Day 2: Find one resource related to it

Day 3: Find a second resource related to it

Step 5: Efficiently use your resources

There’s nothing worse than stockpiling 30 resources and having 100 pages of notes that can go into an essay. How can you possibly synthesize all of that information with the time given for this class essay? (You can’t.)

Rather than reading “Article A” and pulling all the information you want to use into an “Article A Information Page,” try to be intentional with the information as you go. If you find information that’s relevant to Topic 1 in your paper, put the information there on your outline with (article a) next to it. It doesn’t have to be a full citation, you can do that later, but we don’t want to forget where this information came from; otherwise, that becomes a whole mess.

By putting the information into the outline as you go, you save yourself the step of re-reading all the information you collected and trying to organize it later on.

*Note: If you don’t have topics or arguments created yet, group together similar ideas and you can later sort out which groups you want to move forward with.

Step 6: Do Some Self-Checks

It can be useful to use the Pomodoro method when writing to make sure you’re taking an adequate number of breaks. If you feel like the 25 min work / 5 min break routine breaks you out of your flow, try switching it up to 45 min work / 15 min break. During the breaks, it can be useful to go through some questions to make sure you stay productive:

  • How long have I been writing/reading this paragraph?
  • Does what I just wrote stay on topic?
  • Have I continued the "write now, edit later" mentality to avoid getting stuck while writing the first draft?
  • Am I starting to get frustrated or stuck somewhere? Would it benefit me to step away from the paper and give myself time to think rather than forcing it?
  • Do I need to pick my energy back up? Should I use this time to get a snack, get some water, stretch it out, or listen to music?

General Tips:

  • If you are having a difficult time trying to narrow down a topic, utilize office hours or reach out to your TA/professor to get clarification. Rather than pulling your hair out over what to write about, they might be able to give you some guidance that speeds up the process.
  • You can also use (and SHOULD use) office hours for check-ins related to the paper, tell your teacher in advance you’re bringing your rough draft to office hours on Thursday to encourage accountability to get each step done. Not only can you give yourself extra pressure - your teacher can make sure you’re on the right track for the assignment itself.
  • For help with citations, there are websites like Easybib.com that can help! Always double check the citation before including it in your paper to make sure the formatting and information is correct.
  • If you’re getting stuck at the “actually writing it” phase, using speech-to-text tools can help you start by transcribing your spoken words to paper.
  • Many universities have tutoring centers and/or writing centers. If you’re struggling, schedule a time to meet with a tutor. Even if writing itself isn’t tough, having a few tutoring sessions scheduled can help with accountability - knowing you need to have worked on it before the tutoring session is like having mini deadlines. Yay, accountability!

Of course, if writing just isn’t your jam, you may also struggle with motivation . Whatever the challenge is, this semester can be different. Reach out early if you need help - to your professor, a tutor, an ADHD coach , or even a friend or study group. You have a whole team in your corner. You’ve got this, champ!

There has been an awful lot of debate lately about the diagnosis of Attention Deficit Hyperactivity Disorder (ADHD) brought on by a thought-provoking article in the New York Times (NYT) by Alan Schwartz followed by a number of interesting and challenging blog posts here at Psychology Today by Allen Frances, Marilyn Wedge, Robert Berezin and others here as well as from many other sites.

In all the rhetoric, it is important to point out that there may be more common ground, at least between most commenters, than folks might think. Whether it is the NYT article or the blog entry by Dr. Frances, there is almost always at least some fine print acknowledging that ADHD exists and that at least some people deserve the diagnosis. On the other side, even the most ardent supporters of ADHD and its treatment acknowledge that overdiagnosis can be a problem, medications can be misused, and the long-term impact of our treatments are not studied as well as they could be. Thus, we are talking about those kids (and now adults) in the middle: the ones who struggle with focus and activity to the point that it causes problems but who aren’t the kids who anybody could easily recognize as literally bouncing off walls.

This observation brings us to the essence of the problem, which is that attention span and activity level are complex dimensional constructs with no clear boundary between normal and abnormal either on the surface level or even possibly when it comes to the underlying neurobiological processes in the brain. Diagnosing someone with ADHD is a little like diagnosing someone as being too tall, except that you don’t get to use a ruler.

A lack of clear cut-offs points or specific biological “thing” to point at on an MRI scan, however, in no way makes ADHD not real. Apply that logic, and everything from most types of pain to autism to hypertension should be called into question too. Remember, we are talking about the human brain here. Yet despite the brain’s astonishing complexity, progress is being made to understand the causes—yes, causes—of ADHD. The level of evidence that genetic factors play a huge role in ADHD and that the behaviors reflect significant and measurable changes in brain structure and function is simply overwhelming. To deny this fact requires someone to simply write-off and dismiss massive amounts of research data that comes from hundreds of publicly funded scientific studies, not from the pharmaceutical companies, as the New York Time article mistakenly implies.

One convenient way to dismiss these studies is to basically view researchers as corrupt mouthpieces of the drug companies. As the Schwartz article points out, the influence of the pharmaceutical industry is huge and we as psychiatrists would be quite naĂŻve to think that their marketing effects everyone except us. As one of the only professions that actually receives training in the mechanisms through which our beliefs and ideas can be undermined by other forces, we of all people should know better.

At the same time, those pointing fingers also need to keep in mind that there are a lot more ways to introduce a bias or conflict of interest than a speakers’ bureau. Yes, receiving money to talk about ADHD from a drug company will introduce bias, but going on the talk circuit about how evil the pharmaceutical industry has become, or writing a book about why ADHD does not exist (or the fuzzy boundary between traits and illness for that matter), or being trained to do one type of treatment but not another, creates a conflict too. Perhaps the ultimate hypocrisy are those elected officials who preach to physicians about how brainwashed they have become from drug companies and then accept massive political donations.

All of us, I think, could do with a bit less stone throwing and bit more honest introspection.

In closing, I would respectfully suggest a few considerations from folks who come from either sides of the issue.

For those who are inclined to doubt ADHD as a valid entity…

  • Keep in mind that because something is subtle or difficult to measure does not make it unreal.
  • Consider the possibility that at least some of the hundreds of studies of ADHD are not biased from the pharmaceutical industry.
  • Know that most parents of children with significant attention problems and hyperactivity are doing a pretty good job and that you may not look so good either if the same child was in your house.

For those people, especially my fellow physicians, inclined to dismiss all ADHD critics as the lunatic fringe….

  • Understand that we are not immune to the influence of money, prestige, or even the embarrassment of not being right all the time.
  • Stay aware of the possibility that someone who is not entirely correct in one area might be somewhat correct in another.
  • Be humble in our acknowledgement of the immense amount of really important things that we still do not understand when it comes to the brain.

I hope this small effort may serve to bridge many concerned and well-meaning people together. Nobody wants to see kids suffering needlessly. Neither does anyone want to see children being plastered with medications they don’t need. It is time to get to work.

Improving the Persuasive Essay Writing of High School Students with ADHD

Students with attention deficit hyperactivity disorder (ADHD) often experience serious academic deficits. One area that is particularly problematic is written expression. This study assesses the effectiveness of a persuasive essay-writing strategy taught by using the self-regulated strategy development model on the writing skills of 3 high school students with ADHD. Results indicate a marked improvement in the number of essay elements, length, and holistic quality of students' essays. Planning time also increased. The students maintained gains at 3-week follow-up. The article also discusses implications for future research and practice.

In Praise of A.D.H.D.

Ten years ago, when my son Nicolai was 11, his doctor wanted to put him on medication for attention deficit hyperactivity disorder. “It would make him less wild,” I explained to my mother, who was then 85. “It would slow him down a bit.”

My mother grumbled. “Look around you,” she said in Yiddish. “Look how fast the world is changing. He doesn’t need to slow down. You need to speed up.”

It was a surprising recommendation from someone who had never learned to use a microwave. But recent research suggests she had a point: Some people with A.D.H.D. may be naturally suited to our turbocharged world.

Today the word “hyperactive” doesn’t just describe certain individuals; it also is a quality of our society. We are bombarded each day by four times the number of words we encountered daily when my mother was raising me. Even vacations are complicated — people today use, on average, 26 websites to plan one. Attitudes and habits are changing so fast that you can identify “generational” differences in people just a few years apart: Simply by analyzing daily cellphone communication patterns, researchers have been able to guess the age of someone under 60 to within about five years either way with 80 percent accuracy.

To thrive in this frenetic world, certain cognitive tendencies are useful: to embrace novelty, to absorb a wide variety of information, to generate new ideas. The possibility that such characteristics might be associated with A.D.H.D. was first examined in the 1990s. The educational psychologist Bonnie Cramond, for example, tested a group of children in Louisiana who had been determined to have A.D.H.D. and found that an astonishingly high number — 32 percent — did well enough to qualify for an elite creative scholars program in the Louisiana schools.

It is now possible to explain Professor Cramond’s results at the neural level. While there is no single brain structure or system responsible for A.D.H.D. (and some believe the term encompasses more than a single syndrome), one cause seems to be a disruption of the brain’s dopamine system. One consequence of that disruption is a lessening of what is called “cognitive inhibition.” The human brain has a system of filters to sort through all the possible associations, notions and urges that the brain generates, allowing only the most promising ones to pass into conscious awareness. That’s why if you are planning a trip to Europe, you think about flying there, but not swimming.

But odd and unlikely associations can be valuable. When such associations survive filtering, they can result in constructive ideas that wouldn’t otherwise have been thought of. For example, when researchers apply a technique known as transcranial stimulation to interfere with key structures in the filtering system, people become more imaginative and inventive, and more insightful as problem solvers.

Individuals with A.D.H.D. naturally have less stringent filters. This can make them more distractible but also more creative. Such individuals may also adapt well to frequent change and thus make for good explorers. Jews whose ancestors migrated north to Rome and Germany from what is now Israel and the Palestinian territories show a higher proportion of the A.D.H.D. gene variant than those Jews whose ancestors migrated a shorter distance south to Ethiopia and Yemen. In fact, scientists have found that the farther a group’s ancestors migrated, the higher the prevalence of the gene variant in that population.

Or consider the case of the Ariaal, a Kenyan tribe whose members through most of its history were wild-animal herders. A few decades ago, some of its members split off from the main group and became farmers.

Being a wild-animal herder is a good job if you are naturally restless; subsistence farming is a far tamer occupation. Recently, the anthropologist Dan Eisenberg and collaborators studied whether people with A.D.H.D. might thrive in the former lifestyle but suffer in the latter. They found that among the herders, those who possessed a gene that predisposed them to A.D.H.D. were, on average, better nourished. Among the farming Ariaal, the opposite was true: Those who lacked the genetic predisposition for A.D.H.D. were, on average, better nourished. Restlessness seemed to better suit a restless existence.

A.D.H.D. is termed a disorder, and in severe forms it can certainly disrupt a person’s life. But you might view a more moderate degree of A.D.H.D. as an asset in today’s turbulent and fast-changing world. My mother, now 95, long ago realized that speed is the essence of our era. Her intuition about Nicolai proved correct, and she has lived to watch her grandson thrive without taking the A.D.H.D. medication she was dead set against.

ADHD Treatment: Parenting the argumentative child

Children with ADHD tend to be more argumentative and have more explosive emotions than those without ADHD. In fact, it is often noted that those with ADHD may feel emotions up to 3 times MORE intensely than those without ADHD. The most challenging part? They often do not have the emotional or social vocabulary to express what they are feeling or why. When you are speaking to your child and you use words like “No” or “Can’t” or “Don’t”, they will often respond in a not so calm way. They may feel like you have told them the world is ending. Your child may also think the task is repetitive or boring. All of this may result in a tantrum or argument. They believe that their tantrum is warranted because they are frustrated. Often times they don’t feel as if there is any other option than to explode with anger. This is due to lower social and emotional awareness which is caused by the diagnosis of ADHD.

How do I parent an argumentative ADHD child?

It does not help to argue back or yell at your child when they are emotionally volatile. Try to use a low, calm voice that will help set the tone further. They will realize that they do not have to yell to get their point across. We like the phrase HALT. This means, don’t have a conversation with your child when you are Hungry-Angry-Lonely-Tired. Truthfully- any time your blood pressure is raised, it is not a good time to talk. Kids can be super sensitive, and kids with ADHD are incredibly sensitive and aware to voice tone, body language, and perceptions. Remember, they feel things 3 times MORE intensely than those without ADHD they may just not fully understand the dynamics- or see both sides. If you ever wonder why your ADHD child complains you are yelling when you are only irritated- this is why.

Another option to try is interjecting by saying something like, “I understand that you are upset, but can I talk now?” Acknowledging your child will make them feel visible. It is important to focus on good listening skills. Listen to your child’s grievances and then interject to say your point on why you need them to listen to you. Set a good example by listening and teach them that listening is just as important as speaking. Often times we find that parents think they are listening to their child but they truly are not. A third option is to discuss in a neutral time, when both you and your child are in good moods. Use a neutral time (and place) to discuss and teach your child how to communicate effectively with others. This relates to HALT that is explained above and takes a lot of practice! By using a neutral time, it’ll help teach your child so they can process the information. This helps because you do not have to yell or raise your voice at them while they are experiencing heightened emotions. In those heightened emotions, the only thing they can hear or want to hear is you agreeing to let them get their way. In that time, you can tell them it’s okay to agree to disagree on certain topics. Teach them early that many people agree to disagree on a multitude of topics; it’s what makes people unique and different. You can try to explain to your child that it’s okay that you want to color with crayons instead of markers like they do, but it does not make you a terrible person. You both can agree to disagree about which art supplies colors better.

You want to encourage your child to speak their mind and feel comfortable doing it, but that does not mean you lose all control. It is important that your child still sees you as mom or dad opposed to as a friend. It is important they still know that you are the one that calls the shots. Even though you are letting them give their point of view when upset, they still need to listen to your final word. You have the authority not your child. However, this balance is easily lost when parenting ADHD and authoritarian parenting styles are not always the right direction.
